LVN Primary function is to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring provider. Duties include: Identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations; Participate in planning and implementing care in conjunction with the RN, in accordance with the POC; Provide care utilizing infection control measures that protect both staff and patient (OSHA); Assure the continuity of care through delivery of quality patient care; Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations; Demonstrate commitment, professional growth and competency. Qualifications: Education: Graduate of an accredited school of vocational nursing Licensure: Current Texas State license as a Licensed Vocational Nurse, current Texas Drivers License Experience: Two years experience as a Licensed Vocational Nurse in a clinical care setting preferred, home health preferred. Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Transportation: Reliable transportation. Duties include: Assess home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations; Implement/develop/document the plan of care to ensure quality and continuity of care; Provide care utilizing infection control measures that protect both the staff and the patient (OSHA); Supervise and provide clinical directions to the HHA/LVN to ensure quality and continuity of service provided; Assure continuity of quality patient care delivered with appropriate documentation; Monitor assigned cases to ensure compliance with requirements of third party payor; Perform on-call responsibilities and provide on-call service to patients/families as assigned; Provide effective communication to patient/family, team members, and other health care profession. Qualifications: Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure: Current Texas State license as a Registered Nurse, current Texas Drivers License Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, home health preferred. Primary function is to provide personal healthcare and related services to the client in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse. Duties include: Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care; Provide necessary skills to assist the patient with safe transfers and ambulation; Provide necessary skill to appropriately report changes and document pertinent information and care provided to patient to ensure continuity of care; Practice accepted infection control principles; Provide a clean, safe and comfortable environment; Provide an environment which promotes respect for patient, privacy and property; Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake; Demonstrate commitment, professional growth and competency by attending 12 hours of inservices per year. Qualifications: Education: High School Diploma Preferred Licensure: Must have current drivers license Experience: At least one year experience, preferred, as a Home Health Aide or Nursing Assistant in a hospital, nursing home or home health agency, or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
